Dear Parents/Guardians of Rising Fourth Grade Students,
As the school year comes to an end, it is time to think ahead to 4th grade! The health program of the Emerson Board of Education follows the recommendations made by the Bureau of Health Education of the American Medical Association. It is also recommended by the Bergen County Society of School Physicians. The program provides for the routine physical examination of pupils to be completed by the family physician. It is the family physician who is familiar with the child and has knowledge of family and personal history.
All 4th grade students are required to submit documentation of a current physical examination. As your child has their annual physical examination, please send a copy to Mrs. Carberry at Memorial School. The physical form can be located here
Elementary Physical Form. A signed copy of the physician’s form is also acceptable
(printouts from the patient portal do not meet the requirement and will not be accepted)
If for any reason you are unable to arrange this examination through your family physician, please contact Mrs. Carberry at 201-599-4178 ext 3600.
We sincerely appreciate your cooperation in submitting this information prior to the start of 4th grade, so your child will receive the maximum benefits of the program and you can enjoy your child’s transition to Villano School!
